INSTRUCTIONS:
Please fill out the form fields below, then click on the “SUBMIT” button at the bottom of the form. You can select more than one program to order by:

1. Clicking on multiple titles in the “Program Title” field below (hold down the ctrl key to select multiple titles), or
2. You can type in a program title name in the “Type in Program Title:” field below (this is useful if you’ve downloaded
....the Loss Prevention media list in .pdf format from the Loss Prevention Media Library home page.)
3. All titles are in VHS Format unless otherwise indicated. Titles available in DVD are marked "(DVD)".
4. To order the free TML-IRP "Back Injury Prevention for Public Entities" training package to keep, please provide
....contact information and select from the TML-IRP Program field.  Programs in the "Lending Library" field are for
....borrowing and returning.

Your order will be processed within two (2) working days. If the program(s) you have requested are currently checked out, a member of our staff will notify you either by e-mail or telephone as to when the program will again be available.

In order for us to maintain the Media Library for all members, please return borrowed items within 21 days.  We recommend that you use a padded envelope or cardboard mailer when returning the materials and mail it at the first class rate.  Return media library materials to:
